If you are already a unitholder of the Capital Direct I Income Trust, you may register for the Distribution Re-Investment Plan (DRIP). If you are not a unitholder yet, please select Regular Investment, or one of the Registered Investment options to open an account.
The Distribution Re-Investment Plan (DRIP) enables unitholders to re-invest the quarterly distributions they receive and purchase additional units of the Trust.
	Complete and sign the Authorization Form.
	Return the signed Authorization Form to us by fax at 604-430-3287 or by mail to 305 - 555 West 8th Ave, Vancouver, BC, V5Z 1C6.

	Complete and sign pages 1 and 2 of the Subscription Agreement
	Contact your NorthShore Credit Union representative for assistance in completing the bank forms.
	Complete only the option below that applies to you based on your province of residency.

If you are not an accredited investor and are purchasing units having an aggregate subscription price of less than $150,000, complete and sign two copies of the Acknowledgement Form.
If you are an accredited investor and are purchasing units having an aggregate subscription price of less than $150,000, complete and sign the Accredited Investor Questionnaire.

If you are not an accredited investor and are purchasing units having an aggregate subscription price greater than $10,000 and less than $150,000, complete and sign:

two copies of the Acknowledgement Form; and
one copy of the Eligible Investor Questionnaire.

If you are an accredited investor and are purchasing units having an aggregate subscription price of less than $150,000, complete and sign the Accredited Investor Questionnaire.
